Our Clinic


Family Practice Nurse Practitioners serve you with an individual primary care service. We can see and treat every member of your family, creating opportunities to develop personal and lasting healthcare relationships.


We care for a wide range of medical concerns from acute illness such as colds and ear infections to chronic conditions such as heart disease, high blood pressure and diabetes.  We additionally provide regular preventative care visits such as annual physicals, pap/pelvic exams and infant/children's check-ups. As clinics have grown larger and larger, providers are being asked to see more patients in a shorter time period.  We believe healthcare does not need to be that way.  The Grove Street Family Clinic brings back the “Family Clinic” atmosphere by providing healthcare on a more individual level.

 The clinic will be closed the week of August 9-13.  There will be a staff member in the clinic to answer the phones.  They can answers questions and process medication refills.  Any urgent care needs will be referred to a local urgent care or emergency room.

Important Update!

Telehealth visits during the COVID-19 response


The rapid spread of the COVID-19 virus (coronavirus) has impacted all aspects of our daily lives and placed a great deal of strain on our healthcare systems. Our priority is keeping our patients and staff healthy. To do so, we must do everything we can to comply with the recommendations to limit person-to-person contact.

Grove Street Clinic is now providing most of our care virtually (telehealth/telemedicine), either by video or phone. This options allows you to continue to get the care you need in the safety of your home or work. We are able to do Telehealth visits for follow up on chronic conditions, illnesses (cough, fever, urinary symptoms), medication follow up, and many other reasons.


We will continue to provide in-person care that is considered medically necessary and cannot be provided virtually.  At this time, most insurances are covering Telehealth appointments. If you do not have insurance or your insurance does not cover the Telehealth appointment, the cost of the appointment is $80 for a 15 min appointment and $100 for a 30 min appointment. Copays, co-insurance and payments will need to be made at the beginning of the appointment.


We are unable to do COVID-19 testing as we continue to lack the proper protective gear for the staff.


Grove Street Clinic will provide for all of your health care needs in the safest way. During this time of uncertainty, we want to reassure you that we are doing everything we can to provide the best care possible.  Please take precautions. Wash your hands regularly; stay at home and away from large groups of people; and cover your cough. We care about you and our community and want to keep everyone safe.

What is a telehealth appointment?

A telehealth visit is similar to an in-person wellness appointment. We are able to do Telehealth visits for illnesses (cough, fever, urinary symptoms), medication follow up, and many other reasons. During the visit you’ll talk about your current health concerns and learn what to do.

• Your visit can take place from a location that is the most convenient for you.

• Your provider will use a video-based platform to talk with you by computer, tablet or smartphone.

• Our telemedicine platform is secure and HIPAA compliant.

• Your photo and voice will not be recorded or stored.

• Your privacy and rights will be respected and ensured.

Preparing for your telehealth video-based appointment

• Prepare notes or have a list of topics you would like to discuss with your provider.

• Please have information ready for your appointment such as your blood readings, blood sugar readings, temperature or weight

• Make sure your device is plugged in or fully charged.

• Find a quiet space for your appointment. Whether you’re at home, work or traveling, choose a location where you have privacy and are less likely to get interrupted.

• Find space that is well lit. Position yourself so that the main light source is in front, lighting up your face.

• Set your camera at eye level. This will make it easier for the doctor to see and engage with you.

• Use headphones if you need privacy or are hearing impaired.

• Have the clinic’s number handy should you have technical issues or get disconnected during your visit.

If you are a new patient to Grove Street Family Clinic, we will need to review and verbally consent/acknowledge to the new patient forms below.

We will email the New Patient Forms that will need to be completed and returned (via email or fax) prior to your appointment.
